Subject: Show Cause Notice
As per attendance record you have been found absent from duty for more than ten days w.e.f 17-07-04 till to date, without any prior approval / intimation to the concerned Head of Department.
Your above said action amount’s to misconduct within the meaning of article 15 (3) (E) of Hindustan Standing Order Ordinance 1968.
You are, therefore, required to reply this show cause notice within seven days of the receipt of this letter. In case you failed to reply within the stipulated period it would be presumed that you have nothing to explain in your defense and ex-parte action will be initiated against you.
